Certain younger people with disabilities
People with End-Stage Renal Disease (permanent kidney failure)

Unfortunately, Medicare rules have tied our caring hands which keeps us from extending our membership model directly to you.
This means that we are currently unable to take care of any patients who have, or are eligible to receive, Medicare or Medicare Advantage.
